Dogs

Here you can enter all relevant data for dogs. Fields with the (o) sign are obligatory.

Registry No. Enter registry number.
Pedigree No. Enter Pedigree number.
Tattooed No. Enter tattooed number.
Supp. doc. completion Enter Support document completion. Users can choose between next options:
  • Incomplete
  • Complete
Source (o) Enter Dog source. You can choose between next options:
  • No data
  • Domestic 
  • Foreign country
  • Shelter ( animals that they have Source- Shelter are colored red. In this case veterinarian know, that this animal need special treatment)
This field is obligatory. If it isn't filled out, an error message appears (see Dog Source must be entered).
Specific signs​ Enter specific signs
Passport No. (o) Enter passport number. This field is obligatory. If it isn't filled out, an error message appears (see Passport number must be entered).
Issue date (o) Enter issue date of passport.  This field is obligatory. If it isn't filled out, an error message appears (see Issue date of  passport must be entered).
Mother ID (o) Enter dog's mother chip number. This field is obligatory. If it isn't filled out, an error message appears (see Issue date of  passport must be entered).
Size (o) Enter dog's size. You can choose between the following options:
  • No data
  • Big
  • Middle
  • Small
This field is obligatory. If it isn't filled out, an error message appears (see Size must be entered).
Hair Length (o) Enter dog's hair length. You can choose between the following options:
  • No data
  • Middle
  • Short
  • Long
This field is obligatory. If it isn't filled out, an error message appears (see Hair length must be entered).
Color (o) Enter dog's color. This field is obligatory. If it isn't filled out, an error message appears (see Color must be entered).
Weight Enter dog's weight.
Supporting documents Enter supporting documents.
Application date Enter application date.
Unsbscribe date Enter unsubcription date.
Unsubscribe ID Enter unsubscription reason code.  You can choose between the following options:
  • Missing
  • Death
  • Euthanasia of a dangerous dog
  • Euthanasia because of medical reasons
  • Removed by authorities
  • Given away
  • Exported
Registered in Central Pet register We tick option if an animal was registered in the central pet register.
Deregistered from Central Pet register We tick the option if an animal was deregistered from the central pet register.
Shepherd We tick this option if the dog is trained as a shepherd dog.
