6VY - Euthanasia

   

Printout 6VY - Euthanasia allows the printout of the statement of the animal owner upon euthanasia. It contains the data of the statement of the reason for euthanasia, information about the animal, and the owner's signature.

Animal Protection Act clearly lists the conditions under which euthanasia of animals is permitted in its 26th article. Some of these also concern pets.

Below we present an example of an euthanasia printout.

DESCRIPTION OF THE CASE

The owner of the animal, Luna Koratka, brought her cat Milly to the veterinary clinic. The cat reached an age when her life functions began to fail. Veterinarian Valerija performed the euthanasia service. The responsible person in the clinic prints out the statement of the euthanasia of the cat, which the owner signs. This is done in the following steps:

  1. Creating the treatment document
  2. Service performed
  3. Printout Euthanasia

1. Creating the treatment document

The responsible person creates the treatment document as shown in the use case Entering the treatment (general).

In our case, it creates document no.: 23-CLI3-000018 with the details of the owner Luna Koratka and the cat Milly.

Continues to add the identifier of the service performed.

2. Service performed

On the tab Service performed adds the identifier for the euthanasia of the cat: EVTANE_CAT_REF.

When the identifier is added to the treatment document, the program opens a confirmation window: The identifier for euthanasia is selected - Do you want to change the status of the animal to Dead?

3. Printout Euthanasia

The responsible person creates the euthanasia printout via the button in the toolbar  (Printouts).

From the selection of options, selects Euthanasia and the type of printout: 6VY - Euthanasia.

A print preview opens.

To print, clicks the button  (Print).

The owner signs the statement, agreeing to the euthanasia of the animal.
