Forth panel is called Accident Details.

Here we enter the data about the accident:

 

Date of Report

We enter the date when the accident was reported.

Date of Accident

We enter the date when the accident happened.

Time of Accident

We enter the time when the accident happened, without the minutes. We enter 99 if we do not know the time.

Place of Accident

From the register we select the workplace where accident occurred:

0 – Unspecified

1 – Usual workplace

2 – Temporary workplace in the same department

3 – Others

Type of injury

From the register we select a type of injury:

Accident Location

We enter the addres where the accident occurred.

Nature of Injury

From the register, we select a nature of injury.

Injured Body Part

From the register, we select injured body part.

Industry

From the register, we select an industry.

Work Process

From the register, we select work process.

Activity at Time of Accident

From the register, we select activity at time of accident.

Cause of Accident

From the register, we select cause of accident.

Type of injury

From the register, we select a type of injury.

Agent

From the register, we select an agent.

Material Agent

From the register, we select which is material agent.

Short Accident Description

We enter a short accident description.

Was First Aid Provided

From the register, we select if the First Aid was provided.

Estimated Sick Leave

From the selected table, we select estimated sick leave.

Investigated By (employee ID)

From the selected table we select employee who investigated the accident.

Internal Investigation By

Is automatically filed when selecting the employee from employees file.
