Type In this field you can select from the drop-down menu the type of the document:

 
Active In this field you can select if a certain document is active or not.
Number In this field you can enter the number of a certain document.
Serial Number In this field you can enter the serial number of a certain document.
Issuer In this field you can enter the office that has issued a certain document.
Date of Issue In this field you can enter the date of issue of a certain document.
Place of Issue In this field you can enter the place where the document was issued..
Validity In this field you can enter the date till when a certain document is valid.
Note In the Note box you can enter some additional text that is connected to a certain document.
Driver's License Categories In the Driver's License Categories you can select categories that the employee has. This function will appear only if you select in the Type field the Driver's License option.
