Forms

The Forms module enables you to prepare and submit forms required by law, and transfer data about the company's employees. Data applications for compulsory social insurance, i.e. for pension and disability insurance, health insurance, parental care insurance and unemployment insurance, are sent on M forms, prescribed by the Regulations on data application forms for pension and disability and health insurance, parental care insurance and unemployment insurance.

The module also enables you to receive sick notes and send requests for compensation via the SPOT (eVEM) portal. For more information, see the following chapters:

The module enables you to access the eVEM portal and Documents panel, where you can create and correct your own form templates.

This chapter presents the following use case:

Register Employee

The Registration for compulsory social security insurance (M – 1 form) must be submitted by the employer after concluding the employment contract. The chapter contains instructions on how to fill out the M – 1 form in PANTHEON all the way to submitting the form to ESS.
