This panel is for tracking your employees' level of disability and dysfunctions.

Disability
In this field you can select from the drop-down menu the level of disability or
  • Group I
  • Group II
  • Group II (work disabled)
  • Group III
  • Group III (work disabled)
  • Affected work ability - the employee has no acknowledged category, but it is necessary that he or she be transferred to another position.
  • No disability - Invalidity Committee of the Category I acknowledges disability, but the Invalidity Committee of the Category II rejects the decision.

Descr. of disability In this field you can enter the description of the disability (text field).
Cause of Disability In this field you can select from the drop-down menu the cause of disability:
- injury at work;
- occupational disease;
- disease;
- injury off work.
Disability occurred on In this field you can select from the calendar the date the disability occurred on.
Dysfunction In this field you can select from the drop-down menu the degree of the employee's dysfunction.

Desc. of dysfunction In this field you can enter the description of the dysfunction (text field).
Dysfunction occurred on In this field you can select from the calendar the date the dysfunction occurred on.
Conclusion In this field you can enter the number of the conclusion. This field is used in the ZPIZ-KID-SDC - Refund Form report Field 1.
Conc. date In this field you can select from the calendar the date, the conclusion has been met.
Disabled employee over prescribed quotaThis option is used in the REK-1 form. Fields 013a, 124, 308 and in the KV_N form in the points 5.1. and 5.3.
Type of Disability In this field you can select the type of disability. The selected information in this field will be transferred to M-1,2,3 forms.
NoteIn the Note section you  can enter you notes or description. It is a text field.
