You access this window in the Menubar | Personnel | Forms | M-1, M-2, M-1/M-2, M-1A. Here you enter data for forms: M-1 - insurance application, M-2 - insurance cessation, M-1A - report employment agreement SORS.

Entries into the forms are divided in two sections:

  • Section 1 - 23 and
  • Sections 24 - 37.

    After filling the the form, you click the 003557.gif  button and choose between the report forms M1 or M2 and open the preview report, and if everthing being OK print the data
